Birth/Death Records


How do I request a birth or death record?

1.   If possible, requests for birth and death records should be submitted by mail or by using the City's drop box,
located at the entrance to City Hall.    
2.   If you appear in person to pick up the record, please bring a completed form with you.
3.   Please download and complete the application for your record.  Scroll down for a link to the form.  
A copy of photo ID is required.
3.   Make checks payable to the City of Grosse Pointe.  Cash payments accepted for in person requests only.
4.   Requests will be processed as soon as possible (for mail requests - processed within 1 day after receipt). 
5.   Records will be mailed back to you, so be sure to include a return address on the application form. 


BIRTH RECORDS

FEE - Birth Record Copies 
First Copy of Birth Record $20.00   Each Additional Copy $ 5.00  (Costs are applied per child, per request)
 
The City of Grosse Pointe maintains birth records for births that occurred either within the city limits or at Beaumont Hospital Grosse Pointe (formerly Bon Secours Hospital).

The Birth Record Request form is available in printable format.

Who is eligible to order a certified copy of a birth certificate?
    Person named on the record 15 years old and older
    Parent(s) named on the active record
    Court of competent jurisdiction
    Legal representative of person named on record
    An individual or agency that has full guardianship
    Heir of deceased person named on record
    State or federal agency

Who is not eligible to order a certified copy of a birth certificate?
    Power of Attorney
    Individuals with Custody
    Agencies without Court Order
    Spouse
    Children of Registrant
    Siblings

    The request form must be completed and submitted by mail, or in person, to the City Clerk's office.
    Applicants must show photo identification when applying in person. No exceptions will be made to this policy.
    Applicants must send a copy of photo identification when submitting a request by mail.
    All fee requirements and the city's mailing address are listed at the top of the printable form.

PAYMENT OPTIONS - Acceptable forms of payment are personal checks or money orders made payable to the City of Grosse Pointe.  Credit Cards are accepted for in-person payments with photo ID, credit card fees do apply.  Cash should be used for in-person payments only and not sent through the mail or submitted in the City drop box.

Where are original birth and death records maintained?

The State of Michigan Vital Records Office maintains all original birth and death records. As an additional source,
vital records can be requested through the State office by mail, office walk-in or online at www.michigan.gov/mdhhs.

It is urgent that I obtain a record. What should I do?
If a birth or death record request is urgent, the State of Michigan Vital Records Office offers expedited service for mail and walk-in customers for a fee. The State of Michigan also offers Federal Express (1-2 next business day delivery) available within the Continental U.S. for an additional fee. This service is only available for online credit card ordering through the State website www.michigan.gov/mdhhs.

The City of Grosse Pointe does not offer expedited or overnight delivery services for birth or death record requests.

How do I correct or change a record?
The Michigan Department of Community Health Vital Records Office must make any correction or change to a Michigan birth or death record. The Grosse Pointe City Clerk’s Office is unable to make changes to a vital record.
